The Fremantle Trust strive to deliver exceptional care and support to residents within our services and we are disappointed that there have been a number of occasions where we have failed to meet the required levels of care at Lent Rise House. As soon as we identified issues at the home we liaised with our partners and families and have put in place an action plan to ensure the home delivers the expected levels of person-centred care.
We are continuing to work with partner organisations, employees, residents and their families to enable open and transparent conversations while the action plan is delivered.
