About the Role

We are seeking a compassionate and dedicated Adults Social Worker to join our team. This role involves supporting adults with diverse needs, including older adults, individuals with disabilities, those experiencing mental health challenges, and adults at risk of abuse or neglect. You will play a crucial role in assessing needs, safeguarding individuals, and enabling them to live as independently as possible.

Key Responsibilities

  • Conduct assessments to determine the needs and strengths of individuals.
  • Develop, implement, and review care plans in collaboration with service users and their families.
  • Advocate for service users to ensure they receive appropriate services and support.
  • Work in partnership with other professionals, including healthcare providers, housing services, and community organizations.
  • Monitor and review the effectiveness of care plans, adjusting them as necessary.
  • Manage a caseload efficiently, adhering to deadlines and ensuring compliance with policies and legislation.
  • Ensure the safeguarding of vulnerable adults through risk assessments and interventions.
  • Maintain accurate and up-to-date records in line with organizational and legal requirements.
