We are seeking a Dynamic, Driven Individual to lead on Activity & Wellness Provision for all individuals & staff that we support.
Full-time hours available
Friendly supportive work environment, the opportunity to become Trained Person-Centred Facilitators.
The Person-Centred Practice & Methodology is also adopted to all members of staff, as we feel it is vital that this practice is embedded on throughout the service.
The role of the Activities Facilitator

As an activities coordinator, you will work with a Person-Centred Consultant to listen and action what matters to people living within our Service,
To ensure Individuals are supported to make decisions and take Safe Person-Centred Risk to live well.
You will create a Personalised Transition Plan & develop personalised meaningful activities plans.
Promoting shared interests between individuals within the service.
Promote & Source unique experiences using a multitude of community sources and networks.
Develop & Explore how assistive technology can promote independence and wellbeing.
Develop & Explore community & voluntary provision.
You will be responsible to devising alternative communication formats to support the unique communication methods of the individuals we support.
Coordinating & Organising experiences within the service around a 24 hours period 7 days a week
You will work in line with national initiatives and learn the skills to evidence person-led practices.
About you
You will need to be computer & tech-savvy and be able to use Word, Excel, PowerPoint Internet & Emails.
Able to create Posters and Pictorial Planners
You will have the drive to support the team in delivering “Outstanding” rated services.
Have a willingness to work with our Person-Centred Consultant to achieve fantastic outcomes for people living in our homes and will action creatively information that people tell you is important to them while keeping people healthy and safe.
Be outgoing to deliver a wide range of opportunities & evaluate the success.
Be practically creative and able to multitask.
Be able to record in line with legislation requirements to show inclusion and services that meet people’s needs.
You will be creative with resources and budgets.
You will explore your local communities to involve community group involvement, and local volunteers.
You will be Driven, Empathetic and Passionate, have great organisational skills.
You will be an “out of the box thinker” with a drive for Innovative Practice.
Have understanding of working in a person-centred way with people living with dementia/ Mental Health Conditions & Other Age-Related Conditions.
