Maternity Cover 6 months contract
Lannock Manor is a rehabilitation service, that supports 12 male service users, with enduring mental health illnesses, some may have mild learning difficulties and a history of substance misuse.
At Lannock Manor we believe that one size does not fit all and that the service we provide must be tailored to each individual person that we support. The details of your role will therefore be determined by the needs and wishes that we support and their rights to determine what happens in their own environment.
As a Support Worker you will be expected to:
Facilitate a high standard of care through written Person Centred Plans for resident’s/people’s or (other forms of planning relating to people with mental health problems) and to implement the elements of the plan as a member of the staff team.
Act as a key worker for individual people where appropriate.
Adhere to Lannock Manor’s policies at all times
Help the implementation of Person Centred Plans and support people in their rehabilitation, daily living and personal care prompting (if required).
Be professional whilst on shift, follow rota’s, allocation sheets and make sure all duties are carried out in a timely manner.
Support people in undertaking activities or development in Lannock Manor and in the community, including employment, education and other opportunities.
Maintain records as required relating to people’s lives, service and individual financial affairs.
Liaise with external parties, carers and relatives as required.
Undertake any other duties commensurate with the level of responsibility of the job.
Your days of work are variable each week. As a full-time member of staff, you will be required to work 39 hours per week, this will consist of Monday to Sunday, day and night shifts and every other weekend.
Day shift 7.45am - 20.15pm and Night Shift 19.45pm – 8.15am.
