We have an exciting opportunity for a Nurse to join our team on a part-time basis at our nursing home in Aylesbury. The main purposes of this role are to achieve desired outcomes for Service Users in a person-centred way, carrying out nursing tasks and exercising clinical judgement in the provision of care. Those tasks should enable people to improve, maintain, or to cope with health problems, and to achieve the best possible quality of life.
Lewin House is a modern and spacious care home with 70 individual rooms positioned across four wings and plenty of other facilities. Our nursing home in Aylesbury was designed for the effective delivery of nursing care, with a specialist nursing unit located in each wing. There is plenty of communal and outdoor space, so everyone can get involved in the varied and stimulating programme of activities on offer. To make our residents feel at home, personal items can be brought along and added to individual rooms.
To be successful in this role, you will need:
· A valid NMC Pin
· Evidence of clinical training
· A minimum of 2 years previous experience working within a nursing home
· Experience of care planning and key working principles
· Understanding of relevant health & safety legislation
· Working knowledge of the Regulatory Framework relating to the care of Older People
Principle tasks will include:
· Providing respectful high-quality nursing and social care and support for people who use Fremantle services in accordance with their individual Support Plans, acting as a role model of excellent practice for colleagues at all times.
· Supporting the staff team to achieve excellent nursing and social care.
· Planning, writing, implementing and reviewing Support Plans with input from the Service User and/or relative/advocate and key members of the care team.
· Ensure that people using services receive person-centred care in accordance with Support Plans that comprehensively meet their assessed clinical and social care needs, and that are reviewed with appropriate input from relatives, other professionals and staff colleagues.
· Practically assist people with tasks of daily living such as communication, social engagement, personal care, mobility and end-of-life care as directed in the Support Plan.
· Support people to take their medication as prescribed and recorded in the individual Medication Administration Record.
· Ensure that people have companionship and social, emotional and leisure support in accordance with their wishes and the Support Plan.
· Ensure that people receive planned or urgent health care whenever required.
· Record essential information in appropriate locations clearly, accurately and immediately.
· Actively promote the service and Fremantle by clear, respectful and helpful communication with all potential customers including people who use services, relatives, health and social care professionals, other officials and contractors, visitors as well as Fremantle colleagues.
· Contribute to the success of the whole staff team by respecting colleagues and participating in team meetings and communications systems.
· Any other duties consistent with the business of Fremantle, the job, and hours of work as may reasonably be required.
