The Fremantle Trust have an exciting opportunity for a Supported Living Worker to join our team on a full-time basis, based at our service in Chesham, Buckinghamshire.

Supported living is a concept that supports people to live independently who may still require help with personal care. The Fremantle Trust provide innovative and person-centred care for adults with a learning disability through a tailored support programme. Our fundamental principle is to give every single person we support the toolkit they need to live life to the full and as independently as possible. The people we support live in their own homes which may mean shared housing, individual flats and general community housing.

Every individual is unique, so with a highly trained, caring, and compassionate team, we ensure people are at the heart of all we do by empowering them to take control of their own lives and feel part of the community in which they live. Encouraging choice, independence and decision-making ensures the needs of each individual come first.

For shared housing and individual flats, staff are on-site 24/7 to provide support.

For people living in the wider community, we arrange convenient times to visit.

We work alongside individuals and their families to discuss and share ideas.

Emergency contact numbers are available at all times for additional or urgent need.

Principle tasks will include:

· Providing comprehensive support to people who use Fremantle services in ways that are centred on the person and as described in their support plan. This may include assistance with personal care and in meeting social, emotional, education and occupation needs.

· Undertake the duties of a key worker with specific responsibilities for providing support to individual people using the service which will include:

- Working in partnership with people who use the service, other appropriate professionals, and relatives to achieve positive outcomes for service users and to help them understand the choices available to them.

- Develop and keep up to date individual Support Plans and other care and support documents.

- Support people to order and take their medication as prescribed and in the Medication Administration Record.

- Support people to manage their personal finances in accordance with the Support Plan.

- Enable people to take a full and active role in their community should they wish this.

- Liaise with the wider community in order to create opportunities for meaningful occupation for people who use services.

- Support people in working with registered social landlords to maintain their tenancies.

- Work with specialist benefits advisors to support people to access all benefits to which they are entitled.

- Support people to access comprehensive health care services.

- Support people to maintain a healthy life style including diet, exercise and life-style choices.

· Record essential information in service users’ daily reports, care and health monitoring charts, accident and incident reports, and service message systems clearly, accurately and immediately.

· Ensure that day-to-day tasks and routines are carried out to contribute to the smooth running of the service as a whole.

· Actively promote the service and Fremantle by clear, respectful and helpful communication with all potential customers including people who use services, relatives, health and social care professionals, other officials and contractors, visitors as well as Fremantle colleagues.

· Contribute to the success of the whole staff team by respecting colleagues and participating in team meetings and communications systems.

· Any other duties consistent with the business of Fremantle, the job, and hours of work as may reasonably be required.
