The Fremantle Trust have an exciting opportunity for a Supported Living Worker to join our team based at our service in Milton Keynes, Buckinghamshire.

Supported living is a concept that supports people to live independently who may still require help with personal care. The Fremantle Trust provide innovative and person-centred care for adults with a learning disability through a tailored support programme. Our fundamental principle is to give every single person we support the toolkit they need to live life to the full and as independently as possible. The people we support live in their own homes which may mean shared housing, individual flats and general community housing.

Principle tasks will include:

  • Providing comprehensive support to people who use Fremantle services in ways that are centred on the person and as described in their support plan. This may include assistance with personal care and in meeting social, emotional, education and occupation needs.
  • Undertake the duties of a key worker with specific responsibilities for providing support to individual people using the service which will include:
  • Working in partnership with people who use the service, other appropriate professionals, and relatives to achieve positive outcomes for service users and to help them understand the choices available to them.
  • Develop and keep up to date individual Support Plans and other care and support documents.
  • Support people to order and take their medication as prescribed and in the Medication Administration Record.
  • Support people to manage their personal finances in accordance with the Support Plan.
  • Enable people to take a full and active role in their community should they wish this.
  • Liaise with the wider community in order to create opportunities for meaningful occupation for people who use services.
  • Support people in working with registered social landlords to maintain their tenancies.
