However, they have now been brought together by the provider Outlook Care.

Pav Hughes, who has worked in health and social care for 10 years, joined Outlook Care as a service manager in Havering and Waltham Forest in 2013.

Jags Kalsi was recently assessed by Outlook Care and now receives a tailored care package.

Pav Hughes chose Outlook Care, which provides care to people with learning disabilities, mental health needs as well as older people, for her twin brother because of its emphasis on “the importance of working with the family, as well as the importance of empowering Jags”.

The family were involved in the recruitment process for Jags’ staff team, with Jags sitting on the interview panel and having the final say in selecting the people who would be supporting him.

Jags’ journey with Outlook Care to date has been very positive, according to a spokesman who said: “He’s made significant progress. Jags’ medication has been reduced, and his behaviours are now managed by comprehensive guidelines and a well-trained staff team. As a result of this, the number of incidents has dramatically decreased. Jags has also achieved a number of his personal goals, including volunteering regularly in a charity shop and completing a number of college courses, including cookery and IT.”

Jags is 'my inspiration in everything I do'

Pav Hughes has found that “learning and experiencing social care through my very close bond with Jags has helped shape me as a professional. He is my inspiration in everything I do – he is always at the back of my mind. I want to lead services that I would feel comfortable for my relative to live in.

“We recently reviewed Jags’ progress, which included our family and staff coming together to reflect with Jags, and all of his achievements over the past two and a half years. We concluded that Jags is safe, healthy, seems happy and that he is enjoying a life and exploring new thing – which is everything we could ask for.”

'Life feels very good' when you 'help change someone's life'

Piotr Rejek, chief executive of Outlook Care, said: “Every once in a while something amazing happens and you can help change someone’s life.

“When you and the company you work for have had a role to play in that, life feels very good.

“It’s always been very important to me to work closely with families and to empower those people we support. It is vital to allow people to have as much control over their own support and care as possible, whilst keeping the lines of communication open with families at all times. Only by everyone being honest and transparent about the support being received or desired and acting upon that can you ensure people are getting the most out of their lives.”
